Buttons don't appear on the title bar on the first launching of Firefox 89 after a clean boot. Any subsequent Firefox window has the buttons, but not the first window.
Reverted back to Firefox 88 and everything was fine.

I've been able to reproduce the issue, not sure though if a fix is possible.
If you force window buttons for Firefox and use custom spacing (via UltraMon Options > Compatibility), the buttons get added to the first instance but are then too far to the left for subsequent instances.

Just as an FYI, the issue occurs even with the current 91.0a1 version of Firefox Nightly.

I'm sorry to hear you're having trouble with Ultramon in Firefox 89! Sometimes, browser updates can cause unexpected issues with certain extensions.
