some applications are leaving the window on the taskbar after they close but only on the second monitor, i have it set so both taskbars show tasks from both monitors, if i close an app it disappears from the first, but it leaves it on the 2nd. doesn't seem to matter which app.

So far I haven't seen this myself, please let me know if you can reproduce this reliably with a specific application or by taking specific actions.
You could also check if changing the taskbar process priority makes any difference. The option is under UltraMon Options > Smart Taskbar.

i can reproduce it daily, with IE or TightVNC etc or most any app, i've uninstalled and reinstalled.
Right now the main monitor has [outlook express][ie][yahoo][avg] second taskbar has [outlook express][tightvnc*][ie*][ie][yahoo][avg]
those marked with * aren't actually running
I have them set to mirror at normal priority
Selecting taskbar enable will reset them back to normal

Are the additional task buttons on the Windows taskbar (the one with the Start menu) or on the UltraMon taskbar?

Unfortunately I have no idea at the moment what would cause this, I would need to be able to reproduce the problem on my system for further troubleshooting.
To check if it might be a compatibility issue, you could run EnumHooks and post the generated log file or send it to support@realtimesoft.com.
BTW, to quickly refresh the taskbar, right-click an UltraMon taskbar and select Refresh. This should have the same effect as disabling and re-enabling the Smart Taskbar.

Yes, refresh does the same thing.
I'll try it without AVG or the synaptics drivers see what happens.
