Set up is.
Monitor 2/3 - Radeon 9700 (non pro)
Monitor 1 - PCI Rage II
Setup placement is, Monitor 2, Monitor 1, Monitor 3
Monitor 2, will not have a smartbar on it, even when in two monitor mode and monitor
Why?
When monitor 2 is on primary (has windows bar) Monitor 3 will not have a smartbar on it.
I have exited ultramon, restarted it restarted windows etc...

That's most likely a configuration issue, resetting the Smart Taskbar configuration should fix the problem.
To do this, close UltraMon, then run regedit.exe and delete the registry key 'HKEY_CURRENT_USER\Software\Realtime Soft\UltraMon\Smart Taskbar'.
The next time you start UltraMon, a new default configuration will get created for the Smart Taskbar.
