After having had several windows open, I closed some of them, but the smart taskbar didn't update. This used to happen from time to time in the previous versions, but a right-click/Refresh would clean all of that up. I see that's not in the beta right now, though.
Unfortunately, I can't get these entries to disappear. There are duplicates of some of them as well. For example, I have three identical IM buttons, but only one of them functions (I have another on the primary taskbar, but that's a known issue already).
Potentially interesting detail: This started happening much more during a very long install of another product. Memory usage was very high and odds are good the system was swapping a lot.
I can provide a screenshot if that would help.

Yeah, I get this too. If it gets really bad, turning smart taskbar off and then back on should clear it up.

I'm having an issue w/Smart Taskbar in the beta64 version as well but it sounds different. Instead of seeing multiple instances of an app, I'm seeing multiple Smart Taskbars. As I write this, I have 7 Smart Taskbars displayed on my secondary display (only one of which shows the apps that are running). I can right click & show toolbars/change position but that's it.
The only way to fix the problem is to disable & reenable Smart Taskbar in the Ultramon options dialog. I have a screenshot as well if it's needed. Let me know.

What causes the additional taskbars to get added? For example changing display settings, starting an application, etc.

It appears to happen when I dock my laptop (hp nc8430) but the root cause could be anything that I do while I'm not docked. The extra bars only appear on my secondary display and that only shows when I'm docked. I can provide screenshots/registry keys if needed.

The Smart Taskbar settings might be useful, to save them to a file run regedit.exe, then right-click the key 'HKEY_CURRENT_USER\Software\Realtime Soft\UltraMon\3.0.0\Smart Taskbar' and select Export from the menu.

I can't really pinpoint what situations it happens in, it seems to just randomly get cluttered over time. Sometimes I'll see two icons on one taskbar for one app, and sometimes an app on one monitor will show up on both taskbars.
Right clicking and selecting "Refresh" used to correct both taskbars instantly, but that feature is now gone. However, I've added the Smart Taskbar option to Ultramon's right click menu, and clicking "Enable" under that reloads both bars and instantly fixes the issue when it gets really bad. Using that as a quick fix for now.
One example I can give you though where it happens constantly is with Firefox. When I have an FF window open on each screen, the window on the secondary monitor often shows up on both taskbars, and when minimized, the animation indicates it minimizes to the primary monitor, but still remains on both taskbars. If I manually drag the window from one window to the other, it then refreshes and shows up properly on the right taskbar.
I use the "switch window to other monitor" button heavily, dragging only when I have to, don't know if that helps.
