Installed new 3.4.0 and windows minimize icon and ultramon icon are on top of each other at the top of all my apps. Ultramon is pretty unusable at this point.

You'll get this with some applications which use custom window title bars, for example Office 2016, to fix this go to UltraMon Options > Compatibility, then configure settings for the affected application and set the custom button spacing option. I would start with 50 pixels and adjust if necessary.
If this also happens with Microsoft Edge and File Explorer let me know.
