When Visual Studio Code is maximised the UltraMon buttons are on top of the windows ones.

To fix this, go to UltraMon Options > Compatibility, then configure settings for Visual Studio Code, check the option 'force window buttons' and set the custom button spacing option, 130 pixels worked fine on my system.
